Job Overview

LPN

Full-Time 10:45pm - 7:15am.

Duties and Responsibilities

  1. Act in the capacity as a Team Leader in a modified primary care setting.

  2. Provide regular resident status updates to appropriate personnel.

  3. Supervise Nurse Techs. Participate in Nurse Tech evaluations.

  4. Conduct and document a thorough assessment of each resident’s medical status upon admission and throughout the resident’s course

of treatment.

  1. Assist in the implementation of an individualized treatment plan for each assigned resident.

  2. Comply with evaluation, treatment and documentation of Consulate Health Care guidelines.

  3. Complete required documentation in an accurate and timely manner.

  4. Attend and participate in department/facility meetings, as required.

  5. Act in compliance with Consulate, regulatory, and professional standards and guidelines.

  6. Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical competency.

  7. Adhere to facility policies and procedures and participates in facility quality improvement and safety programs.

  4. Assist as a preceptor to train Clinical Nurse I or another II.

  5. Provide routine nursing services for residents as directed.

  6. Must complete Preceptor Training.

  7. May be trained and assigned to perform the Customer Care Liaison duties as needed.

  8. Perform other duties, as assigned.

Working Conditions

Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, Nurses’ stations, resident rooms, etc.).

Moves intermittently during working hours.

Is subject to frequent interruptions.

Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.

Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.

Communicates with the medical staff, nursing personnel, and other department directors.

Works beyond normal working hours, and in other positions temporarily, when necessary.

Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).

Attends and participates in continuing educational programs.

Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants,

tobacco smoke, and other air contaminants.

Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and AIDS and Hepatitis B viruses.

May be subject to the handling of and exposure to hazardous chemicals.

Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents’ needs.

Education & Certifications

Graduate/completion of approved/accredited school of nursing. Valid license as an LPN in the state of practice.

Must have CPR Certificaitons
